When selecting a STAR and Approach in the FMC, everything works as expected if the Approach is selected, followed by the STAR.

If the STAR is selected first, followed by the approach (which is how I usually do it) there is no option to Insert into the Flight Plan - you can just go back to the route and erase your selections.

Is this realistic? I do not believe this was how it worked on the XP version, or with the A310.

You should always select the runway/approach first.  This allows the FMC to filter out STARS not authorized for that runway.
